• Copy of the child's birth certificate.
• Copy of the child's Baptismal certificate.
• Child's immunization record and NYS Physical form
• A check, payable to St. Patrick School in the amount of Registration fee. This is a one-time fee and is not applicable to tuition, for new registrants. It is also non-refundable.
• St. Patrick's parishioner, your fair share envelope number is needed.
• Transfer students, a copy of most recent report card, and standardized test scores.

New York State law mandates "that prior to entering school every pupil K-8 must have received the following immunizations":

• 5 doses of DPT (or DtaP)
• 4 doses of OPV (or eIPV)
• 2 doses of MMR (or 1 MMR and 1 measles)
• 3 doses of Hepatis B
• 2 doses of Varivax (chickenpox)
